Changes effective December 1, 2022.

We’ll continue to accept this form for short-term disability (STD) claims for plan members who:

  • have tested positive for COVID-19 and
  • have an absence of 10 days or less.

We require a completed Attending Physician’s Statement as part of a new STD claim submission for plan members who:

  • have tested positive for COVID-19 and are absent from work for more than 10 days or
  • have flu-like symptoms and have not tested positive for COVID-19
